Zusammenfassung
Bei einem 33-jährigen homosexuellen Patienten ist 7 Jahre zuvor eine HIV-Infektion festgestellt worden. In der Zwischenzeit ist keine kombinierte antiretrovirale Therapie erfolgt. 3 Monate vor stationärer Aufnahme entwickelt der Patient mäßige Durchfälle. Die Untersuchung zahlreicher Stuhlproben auf enteropathogene Bakterien, atypische Mykobakterien, Rotaviren, Adenoviren und CMVerbringt keinen pathologischen Befund. Mit der Kinyoun-Färbung können vereinzelt Kryptosporidien nachgewiesen werden. Unter einer Therapie mit Paromomycin (3-mal 2 g/Tag) bessert sich anfänglich die Durchfallsymptomatik. Während der nächsten Wochen aber nehmen die Durchfälle erheblich zu (>10 Stuhlgänge/Tag), was zu einem Gewichtsverlust von 18 kg führt. Zusätzlich klagt der Patient über eine chronische Sinusitis mit einem glasig-zähen, nichteitrigen Nasenschleim. Zur weiteren Abklärung wird der Patient stationär aufgenommen.
