Zusammenfassung
Epidemiologische Studien zeigen überwiegend einen günstigen Einfluss von moderatem Alkoholkonsum bis zu 20 g pro Tag auf die kardiovaskuläre Mortalität. Dies wird im Wesentlichen auf die Reduktion der koronaren Herzerkrankung (KHK) zurückgeführt. Bei zunehmendem Konsum führt der Anstieg der Mortalität durch andere begleitende Erkrankungen allerdings zu einem Anstieg der Gesamtmortalität. Der experimentell nachgewiesene negative Effekt von Alkohol auf die myokardiale Kontraktilität, die Ausbildung eines arteriellen Hypertonus sowie der Einfluss von Alkohol auf die Inzidenz von Herzrhythmusstörungen kann bisher ätiologisch nicht abschließend geklärt werden. Es werden u. a. Wirkungen auf die Kardiomyozytenmembran, die Synthese der kardialen Proteine und den Kalziumstoffwechsel diskutiert.
