Zusammenfassung
In Kapitel 2 wurden bereits die pharmakologischen Voraussetzungen besprochen, die für alle regionalen Therapieverfahren gelten. Die intraarterielle Applikation von Zytostatika, aber natürlich auch von Substanzen, die den Blutfluss regulieren, ermöglicht therapeutische Ansätze, die gezielt auf ein Organ oder ein bestimmtes Körperareal gerichtet sind. Bei Substanzen, die eine sehr langsame Eliminationshalbwertszeit haben, fallen die Konzentrationen nur sehr langsam ab, und das betreffende Zytostatikum hat damit über einen längeren Zeitraum die Chance, von einem Tumor oder seinen Metastasen aufgenommen zu werden. Die kurzfristig hohen Wirkspiegel, die durch eine meist zeitlich begrenzte intraarterielle Applikation erzielt werden, werden bei solchen Substanzen meist nicht zu einer wesentlich verbesserten Therapieeffektivität führen. Entsprechend der von Collins (1984) angegebenen Formel
mit Rtarget für den regionalen Konzentrationsvorteil und Q für die Austauschrate/Durchblutung, dürfen nur Substanzen mit einer hohen Gesamtclearance (CITB) gewählt werden, und die Durchblutung sollte möglichst niedrig sein.
