Zusammenfassung
Eine medikamentöse Therapie maligner Neoplasien ist an zwei Voraussetzungen geknüpft: Einmal muss das betreffende Medikament prinzipiell wirksam sein können und zum anderen muss das Prinzip der Dosis-Wirkungs-Beziehung bedacht werden. Dies bedeutet, dass ein Medikament mit steigender Dosis eine steigende biologische Wirkung entfaltet. Gleichzeitig steigt aber auch das Risiko von unerwünschten Nebenwirkungen, so dass besonders bei der Therapie maligner Neoplasien die Dosierung so gewählt werden muss, dass eine möglichst günstige Relation zwischen Wirkung und Nebenwirkungen besteht. Bei den meisten Zytostatika ist die therapeutische Breite, also der Bereich, in dem mögliche positive Effekte erwartet werden dürfen, ohne dass nichttolerable Toxizitäten auftreten, gering. Oft ist man gezwungen, für einen bescheidenen therapeutischen Gewinn auch stärkere Nebenwirkungen in Kauf zu nehmen.
