Zusammenfassung
Die rasante Entwicklung von Operationstechniken im Bereich der Wirbelsäule in den letzten vier Jahrzehnten hat, nicht zuletzt durch die Einführung der transpedikulären Schraubeninsertion, zu einer enormen Zunahme von operativen Eingriffen geführt. So zeigt eine Untersuchung von Mendenhall [22] für die USA im Zeitraum 1985 bis 1996 eine Zunahme von operativen Eingriffen im Bereich der Wirbelsäule um ca. 200%. Vergleichbare Angaben aus Europa liegen nicht vor, jedoch kann ein ähnlicher Trend unterstellt werden. Wenn auch ein wesentlicher Anteil der Steigerung der Operations zahlen im Bereich der klassischen Bandscheiben- und der Halswirbelsäulenchirurgie liegt, so ist es auch zu einer erheblichen Zunahme von transpedikulären Stabilisationen im Bereich der Brust- und Lendenwirbelsäule gekommen. Basierend auf den Entwicklungen winkelstabiler Systeme existiert aktuell ein kaum zu überschauender Markt an Pedikelschraubensystemen. Dennoch entspannt sich weltweit eine kontroverse Diskussion um wirbelsäulenchirurgische Eingriffe; neben den zum Teil hohen Kosten werden insbesondere die mit der Implantation assoziierten Komplikationen als limitierend betrachtet.
