L.O.V.E. and Quality of Life within the ICU: How can it improve Patient Outcome?

Abstract

Many factors potentially able to influence outcomes in critically ill patients have been described and widely explored. These include, besides therapeutic issues and quality of care [1], the underlying condition of the patient, the acute severity of illness, as well as organizational and managerial issues in the intensive care unit (ICU) [24]. Each of these areas is discussed in this book. However, quality of life of the ICU team, or of the patient or relatives during or after their ICU stay, which are heavily influenced by the quality of the relationship between the team and the patients/relatives, and could theoretically influence patient outcomes or be responsible for severe psychological sequelae, have been poorly studied. For simplification, we will regroup all those managerial, sociologie, and behavioral issues in the ICU under the term L.O.V.E. (Leadership, Ownership, Values and Expectations). L.O.V.E. will of course be influenced by many factors related to human polymorphism, occurrence of acute and painful events, before or during the ICU stay, but mostly by the managerial and governance profile within the unit, which determine the respect that people (both patient, relatives and health care workers) ‘feel’ during the part of their life that they spend in the ICU. Although very scarce in the hospital setting in general, a few data are available in the ICU [410]. More extensive data are available from the industrial world.
