Summary
In recent years, the number of the patients with crucial ulcer due to several kinds of venous diseases is now increasing also in Japan. Some few patients show severe-grade vein valve insufficiency and refractory crucial ulcer over a long period of time. For such patients, surgical treatment such as direct vein valvuloplasty, or transplantation of the vein valve in addition to vein stripping should be required to repair vein valve function.
At the time of these operative procedures, angioscopy as well as descending venography is necessary to confirm the vein valve function after the repair. These patients showed rapid improvement of the crucial ulcer.
Zusammenfassung
Die Anzahl der Patienten mit Venenkrankheit ist nun in Japan zunehmend. Daher gibt es seit längerer Zeit viele Patienten mit schwergradiger Venenklappeninsuffizienz und therapieresistenten Unterschenkelgeschwüren. Für solche Patienten musste man chirurgische Behandlungen der direkten Venenklappenplastik durch Längsinzision der Vene oder Venenklappentransplantation durchführen, um schlecht heilende Unterschenkelgeschwüren vollkommen auszuheilen.
Für eingehende Beurteilung der Klappenkoaptation nach Venenklappenplastik oder Venenklappentransplantation, musste man Angioskop mit Sicherheit und auch die deszendierende Venographie benutzen. Dadurch kann man endlich schwergradige Unterschenkelgeschwüre ausheilen. Allen Patienten ist es nach den Eingriffen gut gegangen.
