Frakturversorgung am Humeruskopf mit der winkelstabilen PHILOS Platte

Summary

Since January 2002 69 patients with an average age of 72 have been treated with the new angular-stable screw fixation plate (PHILOS). To date 36 patients passed a follow up of a mean interval of 9 months (6 to 15). The mean constant score was 76 points; 59% of the patients archieved exellent or good results. In X-ray analyses implant displacement were seen in 5 (8%) cases with malunion, due to an incorrect placement of the scews. Correct anatomical reduction was achieved in 64%, minimal axial deviation (< 10°) was noted in 26%. No infection or pseudarthrosis was recorded; 2 patients developed an avascular humeral head necrosis. The new implant provides superior stability of fixation of humeral head fragments and reduces the risk of secondary fracture displacement.

Zusammenfassung

Seit Januar 2002 wurden 69 Patienten mit einem Altersdurchschnitt von 72 Jahren mit einer neuen winkelstabilien Platte (PHILOS) am proximalen Humerus versorgt. 36 Patienten erreichten mittlerweile die durchschnittliche Nachuntersuchung von 9 Monaten (6 – 15). Der mittlere Constant Score beträgt 76 Punkte. 59% erreichten ein hervorragendes oder gutes Ergebnis. Die Röntgenkontrollen zeigten eine Implantatdislokation mit Varusfehlstellung bei 5 (8%) Anwendungen. Eine anatomisch korrekte Achsenstellung bestand bei 64%; geringere Achsenabweichungen (< 10°) bei 26%. Bis lang trat keine Infektion oder Pseudarthrose auf. Das neue Implantat hat eine sehr gute Stabilität und reduziert das Risiko einer sekundären Dislokation.
