Treatment of postgastrectomy bone disease by sodium fluoride or ibandronate — a prospective randomized study

Summary

Postgastrectomy bone disease affects about half of the patients after gastrectomy, and no established treatment is currently available. We randomized 13 patients to sodium-fluoride (SF) and 12 patients to ibandronate (I) treatment for 12 months, all patients received calcium (800 mg/die) and vitamin D (1000 IE/die) for 15 months. After 15 months, treatment resulted in an increase in bone mineral density (BMD) of the lumbar spine in both groups (SF: 0.98 ± 0.15 vs 1.02 ± 0.15 g/cm2, p < 0.0003, increase 4.4 ± 7.8%; I: 0.95 ± 0.18 vs 1.00±0.11g/cm2, p < 0.0000002, increase 5.9 ±5.5%). However, femoral neck BMD was unchanged after I treatment (0.75 ± 0.18 vs 0.76 ± 0.18 g/cm2, p = 0.12, increase 2.4 ± 7.9%), but significantly decreased after SF treatment (0.79 ± 0.17 vs 0.75 ±0.15 g/cm2, p < 0.0008, decrease -4.4 ±7.7%).

Zusammenfassung

Nach Gastrektomie kommt es in etwa der Hälfte der Fälle zu einer Abnahme der Knochendichte (KD), eine effektive Therapie gibt es bisher nicht. Wir randomisierten 13 Patienten zur Behandlung mit Na-Fluorid (NaF) und 12 Patienten zur Behandlung mit Ibandronat (I) für je 12 Monate, alle Pat. erhielten Calcium (800 mg/die) and Vitamin D (1000 IE/die) für 15 Monate. Nach 15 Monaten war eine Zunahme der KD von LWK1 – 4 feststellbar (NaF: 0.98 ± 0.15 vs 1.02 ± 0.15 g/cm2, p < 0.0003, Zunahme 4.4 ± 7.8%; I: 0.95 ± 0.18 vs 1.00 ±0.11 g/cm2, p < 0.0000002, Zunahme 5.9 ±5.5%). Am Oberschenkelhals war die KD nach Behandlung mit I unverändert (0.75 ±0.18 vs 0.76 ±0.18 g/cm2, p = 0.12, Zunahme 2.4 ±7.9%), nahm aber nach Behandlung mit NaF signifikant ab (0.79 ±0.17 vs 0.75 ± 0.15 g/cm2, p < 0.0008, Abnahme - 4.4 ± 7.7%).
