Summary

One of the most distressing Symptoms for the operative treated patients are nausea and vomiting. Patients are often more disturbed by PONV than by postoperative pain. The inci-dence of PONV is 30%, so that it is regarded as “the big little problem”. Studies demon-strated, that only 28% of the general surgical patients with PONV received a sufficient anti-emetic therapy. On the other hand, good treatment concepts for successful pharmacological therapy of these negative side effects exist. We discuss the etiology, physiology of PONV and the pros and cons of the pharmacological drug groups for successful treatment. There is a need for surgeons to increase their attention on patient-related Symptoms.

Zusammenfassung

Eines der am meisten den operativ behandelten Patienten belästigenden Symptome ist die postoperative Übelkeit und das Erbrechen. Die Patienten fühlen sind vielfach mehr durch PONV belästigt, als durch den postoperativen Schmerz. Die Inzidenz von PONV liegt im Mittel bei 30%, so dass PONV in der Literatur vielfach als „the big little problem” bezeichnet wird. Studien haben gezeigt, dass nur 28% der allgemeinchirurgischen Patienten, die eine PONV-Symptomatik boten, auch tatsächlich eine ausreichende antiemetische Therapie erhielten. Demgegenüber existieren wirkungsvolle pharmakologische Therapiekonzepte zur Vermeidung von PONV. Wir diskutieren die Ätiologie und Physiologie von PONV und die Vor- und Nachteile der einzelnen Pharmaka-Gruppen zur erfolreichen Therapie. Seitens der verantwortlichen Operateure sollte diesem Symptom mehr Aufmerksamkeit entgegengebracht werden.
