Zusammenfassung
Durch die Hämotherapie-Richtlinien ist vorgegeben: „Blutkomponenten und Plasmaderivate sind verschreibungspflichtige Arzneimittel und dürfen nur auf ärztliche Anordnung abgegeben werden. Die Indikation ist streng zu stellen“. Die Bluttransfusion bedarf daher wie jede andere Arzneimittelverordnung auch einer eindeutigen Indikation.Der klinische Alltag zeigt jedoch, dass diese Vorgabe für die Indikationsstellung zur Bluttransfusion wesentlich schwieriger zu erfüllen ist als für „klassische pharmazeutische“ Arzneimittel. Das ist nicht zuletzt dadurch bedingt, dass bei Anämie verschiedene Kompensationsmechanismen greifen,welche den niedrigen Hämoglobinwert, den dadurch reduzierten Sauerstoffgehalt des Blutes sowie das daraus resultierende verminderte Sauerstoffangebot auszugleichen versuchen. Derartige Kompensationsmechanismen fehlen naturgemäß bei Pharmaka.
