Zusammenfassung

Die wesentliche Kompression des N. peronaeus betrifft die Knieregion, d. h.den Eintritt des Nerven in den Peronaeuskanal zwischen den Köpfen des M. peronaeus longus nach exponiertem Verlauf um das Fibulaköpfchen. Ein Kompressionssyndrom am ventralen Sprunggelenk ist sehr selten. Nach möglichen Schmerzen am lateralen Knie wird die klinische Symtomatik durch Paresen der Fuß- und Zehenhebung einhergehend mit einer Gefühlsstörung am distalen lateralen Unterschenkel bestimmt. Die wichtigste Differenzialdiagnose ist das häufigere L5-Syndrom. Die Diagnose wird durch eine neuro- eventuell auch sonografische Untersuchung gesichert. Bei persistierendem oder progredientem neurologischen Defizit erfolgt die operative Behandlung mit im Vordergrund stehender Spaltung der sehnigen Arkade des M. peronaeus longus ventral der Fibula.
