Zusammenfassung
Bei Gesichtsverbrennungen können Narben oder Pigmentstörungen zu Stigmatisierung und lebenslanger Belastung führen. Aufgrund der guten Durchblutung der Gesichtshaut sowie der hohen Dichte von Hautanhangsgebilden liegen bei oberflächlichen thermischen Läsionen günstige Voraussetzungen für eine rasche Wundheilung vor: sie heilen binnen 6 bis 12 Tagen mit guten Ergebnissen aus. Problematischer sind dagegen mittel- bis tiefdermale thermische Gesichtsläsionen. Ist eine Spalthauttransplantation notwendig, sind die kosmetischen Ergebnisse oft mäßig bis schlecht. Überlässt man dagegen diese Läsionen der Spontanheilung, finden sich oft ausgeprägte, ästhetisch störende hypertrophe Narben. Seit einigen Jahren hilft hier die Sprühtransplantation von Hautzellen. Drittgradige Verbrennungen sollten dagegen grundsätzlich frühzeitig nekrektomiert, schonend konditioniert und unter Berücksichtigung von ästhetischen Zonen und Hautspaltlinien transplantiert werden. Diese Eingriffe sind spezialisierten Einrichtungen und trainierten Chirurgen vorbehalten.
