Abstract
This chapter reviews the imaging appearance of cerebral hydatid disease. Hydatid disease includes cystic echinococcus (CE) and alveolar echinococcosis (AE). The World Health Organization Informal Working Group on Echinococcus classifies CE according to its viability into active fertile (CE1 and CE2), transitional (CE3), and inactive infertile cysts (CE4 and CE5). The CE1 type is the most common type of CE that appears as a large well-defined hypodense cyst on computed tomography scan and with hypointense rim on T2-weighted MR images. The CE2 type appears as a unilocular cyst with small, peripherally arranged daughter vesicles. The CE3 type appears as a primary maternal cyst with multiple large daughter cysts producing wheel-like or honeycomb-like structures. The CE4 type appears as a cystic lesion with floating membranes. The CE5 type appears as a calcified cyst. The succinate peak is detected within the cyst at MR spectroscopy. CE is commonly seen in the brain parenchyma but may be detected within the basal cisterns, the extradural space, ventricles, as well in the calvarium. Imaging can be used to detect postoperative complications and recurrence of CE after surgery as well as monitor patients after medical therapy. AE is a rare form of hydatid cyst that appears as solid or multilocular cystic masses with ringlike or cauliflower enhancement. Imaging helps to differentiate CE from cystic lesions and AE from malignancy.
