Ungelöste Aufgaben der kurativen Kardiologie: Stand 1991

Zusammenfassung

Die kurative Kardiologie, insbesondere die invasive, interventionelle, hat in den letzten 20 Jahren ungeahnte Erfolge gezeigt. So konnte durch die Einführung der Bypasschirurgie Ende der 60er Jahre und ihre weitere Entwicklung in den 70er Jahren bei einer Großzahl von Patienten mit schwerer Angina pectoris die Lebensqualität und Arbeitsfähigkeit zurückgewonnen und auch das Leben mindestens über die nächsten zehn Jahre sowohl bei stabiler wie unstabiler Angina verlängert werden (27). Das gleiche gilt bis zu einem gewissen Grad auch für die von Grüntzig Ende der 70er Jahre eingeführte Ballondilatation (13), obwohl hier Langzeitresultate bezüglich der Lebensverlängerung noch weitgehend fehlen (14). Aber auch die Entwicklung neuer antianginöser Medikamente, vor allem der Betablocker und Kalziumantagonisten, sowie der oral zu verabreichenden Langzeitnitrate und deren Einführung zur prophylaktischen Behandlung der Angina pectoris bzw. der kardialen Ischämie hat die Lebensqualität der Patienten mit Angina pectoris bzw. koronarer Herzkrankheit entscheidend verbessert (26). Leider haben aber diese kurativen Maßnahmen die Inzidenz der Krankheit, welche — zumindest mit zunehmendem Alter — die überwiegende Mehrheit der Menschen der westlichen Zivilisationsländer in irgendeiner Form befällt, nicht entscheidend gesenkt.
