Intraoperative thrombolysis as an adjunct to surgery in acute arterial occlusion

Abstract

The introduction of the baloon-tipped catheter by Fogarty et al. (1) in the treatment of acute limb ischemia resulted in improvement both in amputation and mortality rates. However, the excellent results achieved by this method at the early years proved less satisfactory in recent years mainly because of two reasons: firstly, embolic events originate from atherosclerotic heart disease rather than rheumatic as the latter has declined (2, 3) involving advanced age population with associated peripheral arterial disease complicating their management (4). Secondly, a substantial proportion of patients with acute leg ischemia have arterial thrombosis secondary to arteriosclerosis and balloon catheter embolectomy alone is associated with lower limb salvage and higher mortality rate (5, 6).
