Zusammenfassung
Die richtige präoperative Einschätzung des Schweregrades der Erkrankung eines Patienten ist für die Behandlung und weitere Prognose von großer Bedeutung. Wünschenswert ist zu diesem Zweck eine wissenschaftlich fundierte Methode, deren Standardisierung und Reproduzierbarkeit über die subjektive Beurteilung des behandelnden Arztes hinausgeht. Auch für die interne und externe Qualitätssicherung ist die Stratifizierung von Patienten nach ihrem präoperativen Zustand unerläßlich, da ansonsten ein aussagefähiger Vergleich erheblich erschwert wird. Gleiches gilt auch für die Bearbeitung wissenschaftlicher Fragestellungen, wo Vergleiche zwischen Patientengruppen nur unter Berücksichtigung des präoperativen Schweregrades sinnvoll sind. Auch zur effizienten Planung des OP-Betriebes ist die korrekte Einschätzung der zu erwartenden Mortalität und Morbidität von großer Bedeutung. Nur so ist eine angemessene Terminierung des Eingriffes möglich, um z.B. einen Stau auf der Intensivstation zu vermeiden. Schließlich kann und muß auch zur leistungsgerechten Patientenabrechnung die zu erwartende Komplikationshäufigkeit bestimmter Patientenkollektive für die Zuteilung von Ressourcen herangezogen werden.
