Zusammenfassung
Die neu entwickelten non-peptid Angiotensin II Typ 1 (AT1)-Rezeptorantagonisten stellen eine Klasse von sehr wirksamen Medikamenten dar, die das Potential besitzen, die medikamentöse Therapie der arteriellen Hypertonie und der chronischen Herzinsuffizienz zu erweitern. Bei Patienten mit manifester arterieller Hypertonie ist die Langzeittherapie mit Losartan (50 – 200 mg/d) effektiv, um den Blutdruck auf physiologische Werte zu senken und Remodeling-Prozesse im Myokard und dem Gefäßsystem zu verhindern. Studien, die sich auf ein größeres Patientengut stützen und auch das Risiko für kardiovaskuläre Ereignisse einschließen (beispielsweise der LIFE Trial), stehen jedoch noch aus. Bei chronischer Herzinsuffizienz zeigen experimentelle Untersuchungen und Studien mit Patienten nach Myokardinfarkt, daß die Therapie mit dem non-peptid AT1-Rezeptorantagonisten Losartan (25 – 100 mg/d) nahezu äquipotent ist im Vergleich zu ACE-Inhibitoren (beispielsweise Enalapril 20 mg/d). Die selektive AT1-Rezeptorblockade führt zu einer Normalisierung der neurohumoralen Aktivierung, einer Verbesserung der gestörten koronaren und globalen Hämodynamik sowie zu einer Prävention bzw. Reduktion struktureller Gefäß- und Myokardveränderungen. Untersuchungen, die positive Effekte von ACE-Hemmern und von AT1-Rezeptorantagonisten auf die Mortalität bei chronischer Herzinsuffizienz nach experimentellem Myokardinfarkt im Tiermodell vergleichen, zeigen, daß der Einsatz von selektiven non-peptid Angiotensin II-Rezeptorantagonisten vergleichbar effektiv ist zur ACE-Hemmer Therapie.
