Zusammenfassung
In den letzten Jahren hat das Interesse an der Bedeutung der freien Sauerstoffradikale für die Gewebsschädigung bei ischämischen Prozessen beträchtlich zugenommen [1]. Bei der kardialen Ischämie sind möglicherweise die Myocyten und die Gefäßendothelien sowohl Quelle als auch Ziel dieser Radikale, die während der Ischämie und in der Reperfusionsphase entstehen. Einer der Mechanismen der Radikalbildung in der Reperfusionsphase beruht auf der Aktivität der Xanthinoxidase in den Endothelzellen des ischämischen Areals. Die Bedeutung der Xanthinoxidase für die Entstehung von Sauerstoffradikalen und ihre Bedeutung bei den postischämischen Gewebsschäden wird in der Literatur, zum Teil in Abhängigkeit vom ischämischen Organ, sehr unterschiedlich beurteilt [2,3]. Für die kardiale lschämie liegen, wenn auch umstrittene, tierexperimentelle Hinweise vor [4], daß Allopurinol, ein spezifischer Inhibitor der Xanthinoxidase, das Ausmaß des Infarktgebietes wirksam reduziert. Bislang ist beim Menschen noch nicht die Anwendbarkeit dieser Ergebnisse überprüft worden. Da die physiologischen Substrate der Xanthinoxidase die Purine Hypoxanthin und Xanthin sind, möchte ich in diesem Vortrag eine analytische Methode zur Bestimmung von Hypoxanthin und Xanthin vorstellen und die diagnostische Wertigkeit dieser Parameter beim akuten Myokardinfarkt.
