Abstract
There are some basic phenomena of the medical record that underlie its development over the past five centuries or more. The first is that it is a written personal record of the doctor. This implies not only that it was personal to the doctor who created it, but also it was likely to reflect the training, medical knowledge and activity that the doctor experienced. This provides the context of the recording activity.
