Zusammenfassung
Die dramatische Verbesserung der Überlebensrate nach Einführung des Cyclosporin A führte zu einer seit 1980 rasch wachsenden Zahl von Zentren, welche orthotope Herz- und Herz-Lungen-Transplantationen durchführen, und einer exponentiellen Zuwachsrate dieser Eingriffe. Weltweit waren 1989 über 200 Herztransplantationszentren registriert, und die Gesamtzahl der 1980–1989 durchgeführten Herztransplantationen betrug gegen 10000, diejenige der HerzLungen-Transplantationen gegen 500 [10]. Die folgende Darstellung der anästhesiologischen Problematik stützt sich v. a. auf eigene Erfahrungen bei bisher 83 Herz- und 2 Herz-Lungen-Transplantationen am Universitätsspital Zürich (September 1985 — Januar 1990) und die bei Besuchen der Stanford University gewonnenen Erkenntnisse. Nicht berücksichtigt sind die Besonderheiten der Herz- bzw. Herz-Lungen-Transplantationen im Neugeborenen- und Säuglingsalter.
