Zusammenfassung
Prinzipiell wird bei der temporären mechanischen Kreislaufunterstützung zwischen 2 initialen Behandlungszielen unterschieden. Entweder erfolgt der Einsatz primär bis zur Erholung des Myokards oder aber als Brücke zur Herztransplantation. Bei der Indikationsstellung kann des weiteren zwischen Patienten, welche im Rahmen von herzchirurgischen Eingriffen nicht von der Herz-Lungen-Maschine entwöhnt werden können, und Patienten, welche in einem anderen Zusammenhang eine nicht beherrschbare Verschlechterung der Herzleistung aufweisen, unterschieden werden. Die für die mechanische Kreislaufunterstützung zur Verfügung stehenden Systeme und deren Anwendung können nach den verschiedensten Kriterien klassifiziert werden. Bei der Einteilung nach Anwendungshäufigkeit müssen insbesondere die Herz-LungenMaschine, die intraaortale Gegenpulsation, Blutpumpen mit kontinuierlichem Fluß sowie Blutpumpen mit phasischem Fluß unterschieden werden.
