Zusammenfassung
Vorderkammerlinsen (VKL) werden weithin insgesamt als übermäßig komplikationsträchtig angesehen und abgelehnt. Grund dafür sind hauptsächlich die Mißerfolge der Linsen mit Schlaufenhaptik (Leiske, Azar, Stableflex, Dubroff).
Ziel der Studie war es, die Verträglichkeit der Fußplattenhaptik bzw. deren Komplikationen retrospektiv zu analysieren. Es wurde ein Fallkollektiv von 305 sekundär implantierter VKL nach unkomplizierter ICCE-Aphakie gewählt. Da in dieser Gruppe die Implantation als gesonderter Schritt vorgenommen wird, ist sie zur Feststellung linsenbedingter Komplikationen besonders geeignet.
Der Nachbeobachtungszeitraum erstreckt sich über 10 Jahre (im Mittel 4-5 Jahre). Verwendet wurden folgende VKL mit Fußplattenhaptik: 50 starre Kelman II, 234 flexible Kelman Omnifit, 21 flexible Surefit.
Untersucht wurden bester prä- und postoperativer Visus, sowie das Auftreten von Hornhautdekompensation, Ablatio retinae, persistierendes cystoides Maculaödem, neu auftretendes/verschlechtertes Glaucom.
Im Ergebnis ist die Komplikationsrate der VKL mit Fußplattenhaptik extrem niedrig, auch im Langzeitverlauf. Bei der sekundären Implantation ist die geringe Traumatisierung durch die Operation selbst im Gegensatz zum operativen Vorgehen bei der nahtfixierten HKL hervorzuheben.
Die niedrige Komplikationsrate schneidet im Vergleich mit der Komplikationsrate alternativer Verfahren der operativen Aphakiekorrektur sehr günstig ab und zeigt die Leistungsfähigkeit des Prinzips VKL Typ Kelman-Choyce bei adäquater Indikation und chirurgischer Technik.
Meine Ergebnisse bestätigen die umfassende und richtungsweisende Arbeit von David Apple über Vorderkammerlinsen aus dem Jahre 1987 [2, 3].
Summary
Anterior Chamber lenses (ACL) have, in contrast to my experience, a reputation of being overly complication prone. Therefore a majority of surgeons believes they should be abandonned altogether. The main reason for the bad image is the unacceptable high rate of complications with closed loop ACL (Azar, Leiske, Stableflex, Dubroff).
From the very beginning I used exclusively ACLs with footplate haptic type Kelman Choyce.
The aim of this study was to retrospectively analyse the tolerance of rather the possibility of complications of the footplate haptic. A collectiv of 305 secondarily implanted ACL after uneventful ICCE was chosen. This group is especially suited to find out IOL related complications.
The followup time was 10 years, 4-5 years in the middle. The used ACLs with footplatehaptic were: 50 rigid Kelman II, 234 flexible Kelman OF, 21 flexible Surefit. Examined was the best pre- and postop. vision as well as the signs of keratopathy, retinal detachement, persisting CME and glaucoma.
The complication rate is extremely low over the years comparing favourably with alternative methods of surgical correction of aphakia.
In case of scondary implantation the operation is less traumatizing than a sutured PCL.
My results confirm the fundamental work of D. Apple, published 1987 [2, 3].
