Zusammenfassung
Nach mehr als 10 Jahren Erfahrung mit dem Portsystem ist dieses in der sachgemäßen Anwendung als sicher zu bezeichnen und als dauerhafter Katheter anderen Systemen vorzuziehen. Innerhalb der Hämatologie/Onkologie sind die Portsysteme weltweit am häufigsten eingesetzt worden, und für einen in diesem Fach tätigen Arzt ist eine Therapie von Tumorpatienten ohne den Einsatz dieser Katheter bei der Verwendung heutiger Medikamentenkombinationen und -schedules nur schwer vorstellbar. Dabei hat sich auch die Indikation zur Implantation nicht nur auf Patienten mit schlechten Venenverhältnissen vor oder unter Zytostatikagabe beschränkt, sondern zunehmend auf Patienten ausgeweitet, bei denen eine systemische Chemotherapie in zyklischen Intervallen vorgesehen ist, die einen zentralvenösen Zugang erfordern. Bei dieser letzteren Indikation sollte noch unterschieden werden, ob bei dieser Therapie supportive Maßnahmen wie Antibiotika-, Antimykotika- oder parenterale Cyclosporingaben bzw. parenterale Ernährung zu erwarten sind. In einem solchen Falle ist der Einsatz von Doppelkammersystemen vorzuziehen.
