Zusammenfassung
Obwohl die physiologische Rolle von ANP noch keineswegs komplett geklärt ist, weisen die bisherigen Befunde daraufhin, daß dieses in den Vorhöfen des Herzens gebildete Hormon ein wichtiges Glied bei der Regulation der Volumenhomöostase und Elektrolyttoleranz darstellt (3, 16, 17). Aufgrund seiner diuretischen und vasodilatierenden Eigenschaften sowie direkt suppressiven Effekte auf die Reninsekretion bei „high-renin-states“ (9) kann das atriale natriuretische Peptid als Gegenspieler des Renin-Angiotensinsystems angesehen werden. In ersten Studien konnte gezeigt werden, daß bei Störungen der Volumenhomöostase wie z.B. Nieren- oder Herzinsuffizienz die ANP-Spiegel im Plasma erhöht sind (20, 21). Dabei wird angenommen, daß eine rechtsatriale Dehnung (Druck/Volumenbelastung) zur Sekretion von ANP in die Blutbahn und schließlich zur Diurese und Natriurese führt. In Übereinstimmung damit konnten mehrere Arbeitsgruppen zeigen, daß eine enge Korrelation zwischen rechts-(links)atrialen Druckwerten und Plasma-ANP-Spiegeln besteht (18, 19). Ausgehend von Untersuchungen an normalen Ratten, in denen eine signifikante Steigerung von ANP im Plasma nach Volumenbelastung beobachtet wurde (13), stellten wir uns die Frage, inwieweit dieser Regulationsmechanismus bei Herzinsuffizienz noch intakt ist. Warum führen die hohen ANP-Spiegel im Plasma in dieser Situation nicht zu einer adäquaten Reduktion der Volumenüberladung und Steigerung der Diurese? Um diese Frage zu beantworten, untersuchten wir zentrale und regionale Hämodynamik und Plasmaspiegel

vor und nach intravenöser Verabreichung von 1–28 ANP sowie
-
3.
den atrialen ANP-Gehalt in einem Rattenmodell der chronischen Herzinsuffizienz und stellten die Ergebnisse den Daten normaler Kontrolltiere gegenüber.
