Zusammenfassung
Im Rahmen der Kataraktoperation wurde nach Phakoemulsifikation des Linsenkernes ein bimanuelles Saug-/Spülsystem in Verbindung mit dem Sonocat zur Entfernung der Rindenreste angewandt. Das aus ursprünglich einer Einheit bestehende Saug-/Spülhandstück wurde dafür in zwei funktionell getrennte Handstücke unterteilt, so daß über zwei Parazentesen bei 10 und 2 Uhr einerseits die Spül- und andererseits die Saugkanüle eingeführt werden konnte. Die kleinen Sondendurchmesser garantieren eine optimale Abdichtung der Wundlippen, so daß während des Eingriffes unter der Voraussetzung eines dichten korneoskleralen Tunnels ein positiver Druck in der Vorderkammer erhalten bleibt und für den Verschluß der Parazentesen keine Hornhautnähte erforderlich sind.
Dadurch wird der korneosklerale Tunnel beim Absaugen der Rindenreste geschont, und die unabhängige Spülsonde gewährleistet einen sicheren Abstand der hinteren Kapsel von der Öffnung der Saugkanüle. Weiter können alle Abschnitte des Kapselsackes, besonders die bei 12 Uhr, durch Seitenwechsel der Handstücke erreicht werden.
Die operative Technik wird genau beschrieben und eventuelle Komplikationen dargestellt. Der Astigmatismus wurde präoperativ, eine Woche, ein Monat und 3 Monate postoperativ mit dem Ophthalmometer nach Javal gemessen und mit Hilfe der Vektoranalyse ausgewertet.
Summary
After phacoemulsification of the nucleus a bimanual I/A system was used for removal of the remaining cortex rests. The two handpieces of the I/A system were introduced through two paracenteses at 10 and 2 o’clock. The small diameters of the paracenteses guarantee a tight closure of the corneal wound. In no case corneal sutures were necessary.
The irrigation device secures the posterior capsule during aspiration of the remaining cortex with the aspiration device. Furthermore it is very easy to remove the remaining cortex rests at the 12 o’clock position.
The surgical procedure and complications will be demonstrated. The values of the astigmatism were measured with the ophthalmometer of Javal preoperatively, one day, one week, one month and three months postoperatively. All values were analysed by the vector analysis of Cravy.
