Erfahrungen mit temporaler „Clear-Cornea“- Technik unter lokaler Tropfanästhesie

Zusammenfassung

Zwischen Oktober 1992 und Februar 1994 wurden 227 Kataraktoperationen unter Anwendung der „Clear-Cornea“-Inzision von temporal durchgeführt, 167 davon in lokaler Tropfanästhesie. Die Schnittlänge betrug je nach implantiertem Linsentyp (einstückige, kahnförmige Silikonlinse, 3stückige Silikonlinse mit Prolenebügel, 5mm PMMA Linse) 3, 4 oder 5 mm. Wunden bis 4 mm wurden nicht vernäht, 12 von 36 5mm Wunden wurden aus Sicherheitsgründen mit einer radiären Naht versehen. Rechte Augen wurden rechtshändig und linke Augen linkshändig operiert, wobei die Position des Operateurs in allen Fällen am Kopfende des Tisches verblieb. Die Tropfanästhesie gewährleistete dem Patienten eine der Peribulbäranästhesie absolut vergleichbare Schmerzfreiheit. Sowohl die intraoperative Durchführbarkeit wie die postoperativen Ergebnisse im Hinblick auf Wundheilung, Stabilität, Astigmatismusverhalten und visuelle Rehabilitation waren mehr als zufriedenstellend.

Summary

Between October 1992 and february 1994 227 eyes underwent cataract surgery with clear-corneal incision using a temporal approach. 167 cases were done under topical anesthesia. The size of the opening depended from the inserted IOL: 3 mm with a one piece silicone lens, 4 mm with a 3 piece silicone lens with prolene loops and 5 mm with a 5mm PMMA lens. Wounds up to 4 mm remained unsutured, 12 of 36 5mm wounds received 1 radial suture for greater security. Surgery was preformed with the right hand on right eyes and with the left hand on left eyes, the surgeon remaining in all cases at the upper end of the table. Topical anesthesia revealed to be as efficient as peribulbar anesthesia in this procedure. The intraoperative safety and the postoperative results in regard to wound stability and visual acuity were more than satisfactory.
