Neuer Hornhautmarkierer für perforierende Keratoplastiken und refraktive Hornhauteingriffe

Zusammenfassung

Unvorhergesehen hohe Astigmatismen und Immunreaktionen sind die häufigsten Komplikationen nach perforierender Keratoplastik. Für den Astigmatismus sind neben der Hornhautpathologie vor allem die Wundadaptation, die Trepanationsweise bei Patient und Spender, aber auch die Nahttechnik entscheidend. Es wird ein spezieller Nahtmarkierer vorgestellt, der einen exakt reproduzierbaren Nahtverlauf im Sinne der von Hoffmann [3] beschriebenen doppelten Sternnaht ermöglicht. Zusammen mit dem Fadenkreuz des geführten Trepansystems (GTS) ist mit diesem Markierer auch eine absolut exakte Zentrierung des Transplantates möglich.

Summary

Unforeseen high astigmatism and immunologic reactions are the most common complications following perforating keratoplasty. Adaptation of wound edges, and mode of trephination of both donor and patient cornea and last but not least techniques of suturestyle are the factors determining postoperative astigmatism. A new corneal marking device for keratoplasties and epikeratophakias is introduced. This marker helps the surgeon both to center the trephine and to perform a perfect double running torque antitorque suture. Additional single sutures are not necessary. A postoperative astigmatism study with this new device is under way.
