Zusammenfassung
Die Behandlung der Hyperthyreose mit Radiojod ist ein strahlentherapeutisches Verfahren und beruht auf der Applikation strahlender Energie. Die Strahlung entsteht beim radioaktiven Zerfall der Jodisotope. Ihre Energie löst in dem erkrankten Gewebe Ionisationsvorgänge aus, die ihrerseits in den bestrahlten Zellen bzw. Geweben biologische Wirkungen hervorrufen. Stabiles und radioaktives Jod sind nur physikalisch verschieden; chemisch und biologisch verhalten sie sich völlig gleichartig, so daß der Organismus das zugeführte Radiojod genau so behandelt wie das stabile Jod. Diese Tatsache und die nahezu selektive Affinität des Schilddrüsenparenchyms für das Element Jod bilden die grundlegende Voraussetzung für die Radiojod-Therapie. Das mit dem Blut herangeführte Radiojod wird von den epithelialen Zellen der funktionsgesteigerten Schilddrüse begierig aufgenommen und im Kolloid passager gespeichert. Es gelangt demnach unter Ausnutzung der Stoffwechselwege gezielt in das erkrankte Gewebe und bewirkt hier auf Grund der günstigen räumlichen und zeitlichen Dosisverteilung eine optimale Strahlenwirkung. Diese ist abhängig von der Strahlenqualität (Energie, Reichweite, spezifische Ionisation), der Konzentration und Verweildauer der Strahlung sowie von der Strahlenempfindlichkeit des Gewebes und führt zu einer Verminderung der Gewebsaktivität, und darüber hinaus zu einem Abbau bzw. zu einer Zerstörung der Schilddrüsenzellen. Der biologische Wirkungsmechanismus unterscheidet sich also nicht von dem anderer ionisierender Strahlen, z. B. Röntgenstrahlen. Neuartig ist jedoch die Applikation der Strahlenenergie über den Stoffwechselweg, die eine Berücksichtigung der im Einzelfall vorliegenden Stoffwechselverhältnisse verlangt. Die zeitlich begrenzte und mengenmäßig inkonstante Beziehung der Strahlenquelle zur Schilddrüse schafft dosimetrische Probleme, deren Lösung für den Erfolg der Radiojod-Therapie ausschlaggebend ist.
