Nichtokklusive ischämische Enteropathie — Diagnose, Differentialdiagnose und Therapie

Summary

Reduced cardiac output is the pathogenetic principle of non-occlusive disease. Prerequisites for early diagnosis include anamnesis, clinical and laboratory findings, sonography, mesentericography, contrast enema, and coloscopy. Conservative treatment with vasoactive drugs is promising in early stages. As the abdominal symptoms develop latently, laparotomy is indicated in most cases in order to identify necrosis or perforation and to allow surgery according to the intraoperative findings. Despite indisputable progress made during the past years, the death rate in our patients (n = 42) is still 43%. Only early diagnosis and consequential therapy can achieve better results.

Zusammenfassung

Pathogenetisches Prinzip der NOD ist ein „low cardiac output syndrome“. Anamnese, klinisches Bild, Laborkonstellation, Sonographie, Kontrasteinlauf, Koloskopie und Mesentericographie führen zur Diagnose. Im Frühstadium der NOD ist eine konservative Behandlung mit vasoaktiven Pharmaka angezeigt. Das Bild des akuten Abdomens bedingt eine sofortige Laparotomie, die im allgemeinen Nekrosen, Perforationen, u.U. Ein- oder Vierquadrantenperitonitis entdeckt. Die chirurgische Maßnahme richtet sich dann nach dem intraoperativen Bild. Trotz aller Behandlungsfortschritte während der letzten Jahre liegt die Letalität im eigenen Krankengut von 42 Patienten immer noch bei 43%. Nur eine frühe Diagnose und eine konsequente Therapie kann diese Ergebnisse verbessern.
