Zusammenfassung

Die kontinenzerhaltende Proktokolektomie mit ileoanalem Pouch stellt zur Zeit bei der Kolitis ulcerosa unter bestimmten Indikationen und bei der familiären Adenomatosis die Operationsmethode der Wahl dar. Bei der Kolitis ulcerosa erfolgt eine Kolektomie mit transanaler Mukosektomie. Als Dünndarmreservoir wird von uns der J-Pouch bevorzugt, der mit der Analhaut anastomosiert wird. Bei der familiären Adenomatose wird eine subtotale Kolektomie durchgeführt, wobei ein ca. 2 cm langer Muskel-Schleimhautschlauch für eine transanale zirkuläre Klammernahtanastomose mit dem vorgeschalteten J-Pouch erhalten bleibt.

Summary

Currently proctocolectomy, which maintains continence with an ileoanal pouch, is the surgical procedure of choice in familial adenomatosis and in certain indications for ulcerative colitis. In ulcerative colitis colectomy is performed in combination with transanal mucosectomy. As a small bowel reservoir we prefer the J-pouch, which is anastomosed with the anal canal. In familial adenomatosis subtotal colectomy is done; this provides a 2 cm long muscle-mucose tube for a transanal anastomosis circularly autosutured with the J-pouch.
