Summary
The main prerequisites for successful reconstructive venous surgery include (1) critical selection of patients according to morphological and functional diagnostic aspects; (2) optimal surgical techniques; (3) the routine use of intraoperative lumen control in all disobliterative procedures (vascular endoscopy, phlebography); and (4) selective use of protective measures (temporary a. v. fistula, anticoagulans). Expanded PTFE-prostheses with increased microporosity and external ring support are the most suitable substitutes for big veins. Antireflux surgery, such as valvuloplasty, transposition technique and free valvular transfer, is a new approach for the repair of deep venous insufficiency.

Wichtige Voraussetzungen für die erfolgreiche Durchführung rekonstruktiver Eingriffe am Venensystem sind 1. eine kritische Indikationsstellung an Hand einer präoperativen Funktionsdiagnostik und Phlebographie; 2. optimale gefäßchirurgische Technik; 3. intraoperative Lumenüberprüfung bei allen desobliterativen Eingriffen (Gefäßendoskopie, Phlebographie); 4. gezielter Einsatz bestimmter protektiver Maßnahmen (z.B. Anlage einer temporären a.v. Fistel, postoperative Antikoagulation). Expanded-PTFE-Prothesen mit erhöhter Mikroporosität und äußerer Wandverstärkung bieten heute die größte Erfolgschance für den großlumigen Venenersatz. Die sogen. venöse Antirefluxchirurgie (Valvuloplastik, Transposition auf klappenschlußfähige Seitenäste, freier Klappentransfer von einer Oberarmvene) befindet sich bislang noch im Stadium der klinischen Erprobung.
