Summary
A conservative attitude to surgical treatment of Crohn’s disease may endanger patients, lives, especially in acute situations. Indications for emergency operation and intraoperative management are subject to special rules. Resection should be minimal but terminated in a marcoscopically disease-free area. In case of perforation, resection is mandatory; mere sewing closed is inadequate. Subtotal colectomy should be performed in cases of toxic megacolon. Abscesses should be drained using a pig-tail catheter; if the abscesses do not heal, surgery is necessary.

Zusammenfassung
Eine restriktive Einstellung gegenüber einer chirurgischen Therapie beim Morbus Crohn kann Patienten, besonders bei Akutsituationen, erheblich gefährden. Die Indikation zum Akuteingriff wie auch das intraoperative Vorgehen unterliegt bei Crohn-Patienten besonderen Regeln. Bei Resektionen sollte sparsam, jedoch makroskopisch im Gesunden reseziert, bei Perforationen reseziert und nicht übernäht werden. Die subtotale Kolektomie ist beim toxischen Megakolon anzustreben, bei Abszessen sollte zunächst die Pig-tail-Katheterdrainage versucht werden. Erfolgt keine Sanierung, muß die Operation erfolgen.
