Summary
Acute stone-related cholecystitis is treated by early cholecystectomy. Percutaneous cholecystostomy seems more and more the preferred treatment of stress-related acalculous cholecystitis. Diagnosis is confirmed by ultrasound. Simultaneous combined operations, i.e. cholecystectomy and common bile duct revision, are performed in patients under 65 years of age without serious concomitant diseases and if the operation is not hazardous. The inhospital mortality amounts to about 3%.
Zusammenfassung
Behandlungsverfahren der Wahl bei akuter, steinbedingter Cholecystitis ist die Frühcholecystektomie. Bei streßbedingter Cholecystitis tritt zunehmend die perkutane Gallenblasendrainage in den Vordergrund. Die Diagnosestellung erfolgt klinisch, ihre Sicherung durch Sonographie. Bei Patienten unter 65 Jahren, Fehlen gravierender Begleiterkrankungen und übersichtlichen anatomischen Verhältnissen wird die simultane Choledochusrevision bei Choledocholithiasis empfohlen. Die Hospitalletalität bei Frühcholecystektomie beträgt etwa 3%.
