Zusammenfassung
Stent-Implantation hat die Behandlung der koronaren Herzkrankheit revolutioniert. Während der Ausgang nach Ballonangioplastie auch unter optimalen klinischen und angiographischen Bedingungen unbestimmt ist, der Erfolg kann vom Stent-ähnlichem Bild bis zur notfallmäßigen Koronarchirurgie reichen, sind die Ergebnisse mit Stent-Implantation vorhersehbar. Dies schafft eine große therapeutische Sicherheit. Während mit Ballonangioplastie immer die Gefahr eines akuten Gefäßverschlusses und einer notfallmäßigen Koronarchirurgie besteht, ist diese Komplikation mit der Stent-Technik sehr selten geworden. Während die Ballonangioplastie durch verschließende Dissektion, thrombotischen Verschluß und chronische Restenosierung kompliziert ist, ist mit der Stent-Technik grundsätzlich nur noch die chronische Restenosierung verblieben, da auch die subakute Stent-Thrombose bei korrekter Indikationsstellung, bei optimaler Implantationstechnik und adäquater prophylaktischer antithrombotischer und antithrombozytärer Therapie praktisch nicht mehr existiert. Mit den beiden großen Vergleichsstudien Ballonangioplastie gegen Stent-Implantation Benestent (6) und Stress (3) konnte die Überlegenheit der Stent-Technologie erstmals belegt werden. In beiden Studien fand noch das herkömmliche antikoagulatorische Behandlungsschema mit Heparin, Dicumarol, Dipyridamol, ASS und Dextran Anwendung. Trotz intensiver gerinnungshemmender Vorsorge wurde eine subakute Stent-Thrombose bei 3,5 % und bedeutende Blutungskomplikationen bei 14,6 % der Patienten beobachtet. Damit war die Technik nur sehr eingeschränkt anwendbar, da unter diesen Umständen für Blutungskomplikationen und ihre Vermeidung hohe Kosten in Kauf genommen werden mußten, insbesondere für Medikamente, Laboruntersuchungen, gefäßchirurgische Eingriffe und Bluttransfusionen.
