Zusammenfassung
Bei strukturell intaktem Endokard ist unter physiologischen Blutströmungsbedingungen keine intrakardiale Thrombenbildung zu erwarten. Das aus einer durchgehenden Einzelzellschicht bestehende endokardiale Endothel stellt eine funktionelle Barriere zwischen dem Blut und den ausgesprochen thrombogenen subendothelialen Geweben dar (6), so daß ein strukturell und metabolisch nicht verändertes Endokard keine Aktivierung der plasmatischen Gerinnungskaskade bewirkt und zellulären Blutelementen keine Adhäsion erlaubt. Es verfügt darüber hinaus über eine beträchtliche Synthesekapazität, vermag erhebliche Mengen von von-Willebrandt-Faktor zu speichern und kann so seine Funktion als athrombogene Oberfläche erfüllen. Störungen der funktionellen Integrität des endokardialen Endothels, z.B. im Gefolge immunologischer Prozesse (rheumatisches Fieber) oder mechanischer Schädigungen bei veränderten intrakardialen Blutströmungsbedingungen (turbulenter Fluß im Gefolge von Stenosen oder Regurgitationen), resultieren in einer erhöhten Rate intrakardialer Thrombenbildung mit der Gefahr konsekutiver Embolien (12, 13). Neben Endokardläsionen und z.B. strömungsbedingten Strukturveränderungen des Endokards prädisponieren die Implantation prothetischen Materials, durch die das umgebende oder bedeckende Endokard stets besonders stark strukturell verändert wird, und vielfältige sonstige Störungen der kardialen Morphologie und Physiologie zur Bildung intrakardialer Thromben (Tabelle 1). Hierbei spielen im Niederdruckgebiet vor allem verminderte Blutströmungsgeschwindigkeiten und niedrige Scherkräfte mit Ausbildung von Arealen relativer Blutstase, im Hochdruckgebiet erhöhte Blutströmungsgeschwindigkeit mit Übergang in turbulente Strömungen, hohe Scherkräfte und konsekutive Zellschädigungen eine Rolle (6, 12).
