Zusammenfassung
Während sich in früherer Zeit die Untersuchung von Störungen im Kohlenhydratstoffwechsel mit Hormon-Belastungsproben auf die Anwendung von Insulin und Adrenalin beschränkte, hat in den letzten Jahren die Anzahl der Belastungsteste durch Verabreichung von Hormonen einen deutlichen Anstieg erfahren. Ein besonderes Interesse erwecken dabei die Teste, die auf der Provokation von Kohlenhydrat-stoffwechselstörungen durch Nebennierenrindenhormone und ihre Derivate beruhen und die die Anwendung bei einer frühzeitigen Erkennung der Glukoseintoleranz finden könnten. Zu diesem Zwecke wurden auch Belastungsteste eingeführt, denen eine Stimulation der Langerhansschen Inseln durch Sulfonylharnstoffderivate zur erhöhten Insulinabgabe zugrunde liegt. Diesen Proben wird im Vergleich mit dem peroralen Standard-Glukose-Belastungstest eine größere Empfindlichkeit bzw. Spezifität zugeschrieben. Allerdings ist die Beurteilung der Brauchbarkeit dieser Teste zur Frühdiagnose des Diabetes mellitus nicht einheitlich. Es ist daher wohl am richtigsten, diese Belastungsteste noch immer als im Untersuchungsstadium befindlich anzusehen, obwohl schon viele Beobachtungen ihrer Anwendung vorliegen. Eine endgültige Beurteilung ihrer Verwendbarkeit wird erst möglich sein, wenn eine signifikante Beziehung zwischen den gefundenen positiven Ergebnissen und der Manifestation der Störungen im Kohlenhydratstoffwechsel an hinreichend großen Bevölkerungsgruppen, in einer hinlänglich langen Zeit verzeichnet sein wird. Im Hinblick auf die große Bedeutung, die einer Frühdiagnose latenter Störungen im Kohlen-hydratstoffwechsel zum Zwecke der Diabetes-Prophylaxe zukommt, verdient jeder diesbezügliche Versuch Beachtung, erfordert aber gleichzeitig eingehende Beurteilung.
