Zusammenfassung
In der Bundesrepublik Deutschland ist von 1981 bis 1987 die Zahl der jährlich durchgeführten Herzkatheteruntersuchungen von 40655 auf 114040 gestiegen. Der Anteil der Koronardilatationen erhöhte sich im gleichen Zeitraum von 1,2% auf 10,6% [5], ohne daß ein Ende dieser Entwicklung abzusehen wäre. Einerseits haben die Erfahrung der Untersucher und die Qualität des verwendeten Materials deutlich zugenommen, andererseits sind für perkutane transluminale Angioplastien (PTCA) weitlumige Gefäßschleusen bis 8 Charr (2,7 mm) erforderlich, die über größere Gefäßläsionen eine höhere Komplikationsrate erwarten lassen. Darüber hinaus macht nach PTCA die relativ hohe Restenose-rate von 20–40% häufig Kontrollangiographien erforderlich, bei denen der Gefäßzugang oft durch teilresorbierte derbe Resthämatome und Narben erheblich erschwert wird.
