Aktuelle Therapie des Oesophaguskarzinoms

Zusammenfassung

Die Diagnose des Oesophaguskarzinoms erfolgt überwiegend spät. Wird eine progressive Dysphagie apparent, ist erfahrungsgemäß bereits mehr als die Hälfte des Speiseröhrenlumens durch Tumor eingeengt. Diagnose und kurative Resektion eines Frühkarzinoms der Speiseröhre sind daher selten. Earlam prognostizierte 1980 auf der Basis multipler Publikationen eine irresektable Situation für 40% der Patienten mit Oesophaguskarzinom bereits zum Zeitpunkt der Diagnosestellung [5]. In der Chirurgischen Univ. Klinik Mannheim betrug die Resektionsrate für das Oesophaguskarzinoms in den Jahren 1973 bis 1986 nur 59%, für die Zeit von 1980–1986 aber schon 65% [15]. Überwiegend bestand zum Operationszeitpunkt ein Stadium T3, N1.
