Zusammenfassung
Über die Effizienz der Vagotomie in der Behandlung der gastroduodenalen Ulcera herrscht allgemeine Zustimmung. Gleiches gilt für die Effektivität des Säurereduktionspotentials der verschiedenen Vagotomieverfahren [4, 5, 12, 14]. Das Ausmaß der Reduktion der Säuresekretionskapazität des Magens nach Vagotomie wird noch diskutiert. Donovan und Mitarbeiter (1979) fanden keinen signifikanten Unterschiede der Säureminderung nach proximaler gastraler und trunkulärer Vagotomie, d. h. daß das Säurereduktionspotential der beiden Vagotomieformen gleich ist. Nach anderen Autoren wird die Reduktion der Säurekapazität nach Vagotomie im Schrifttum mit 60–100% beziffert [3, 12, 17]. Aufgrund derartiger Ergebnisse wird nicht selten nach trunkulärer Vagotomie dem Faktor Säure eine untergeordnete Rolle zugeschrieben. Kaum analysiert ist das Verhalten der Säurekonzentration im Magensaft nach Vagotomie. Unter Berücksichtigung der multifaktoriellen Pathogenese des Ulkus ist es von klinischer Relevanz welche Azidität nach Vagotomie erreicht wird. Früher wurde das Auftreten pathologischer Schleimhautläsionen nach Vagotomie anderen Faktoren zugeschrieben, die Säurekonzentration fand keine Berücksichtigung [1, 2, 6, 18, 19]. Unlängst wurden aus Japan Beobachtungen über peptische Ulcera im hochgezogenen Magen nach Oesophagektomie mitgeteilt [22]. Mit diesen Überlegungen und der differenten Einschätzung der Vagotomie im Hintergrund galt die Zielsetzung der Erfassung der pH-Werte im Magensaft nach Oesophagektomie.
