Zusammenfassung
Es hat in der Vergangenheit nicht an Versuchen gefehlt, nach Möglichkeiten zu suchen, die Behandlungssituation beim Oesophaguskarzinom durch Ausdehnung und Ausweitung operativer Therapie zu verbessern [4, 7, 12]. Die Ergebnisse sind aber nach wie vor enttäuschend. Die schlechten Resultate werden unter anderem durch eine hohe Rate primär inoperabler Tumoren, aber auch durch frühzeitiges Auftreten von Lokalrezidiven sowie Fernmetastasen bedingt. Nicht zuletzt darf die immer noch relativ hohe Mortalität der operativen Therapie des Oesophaguskarzinoms außer Acht gelassen werden. Ansätze zur Verbesserung der Therapieergebnisse sind daher zum einen in einer weiteren Standardisierung des operativen Eingriffs, in einer Verbesserung der Risikoabschätzung und vor allem auch in einem multimodalen Behandlungskonzept zu sehen.
