Zusammenfassung
Nierentumoren haben bekanntlich die fatale Eigenschaft, frühzeitig in das venöse Gefäßsystem einzubrechen, um sich hier als intraluminale Zapfen per continuitatem zentripedal zu entwickeln. Dies tritt in gut einem 1/4 der Fälle ein. In 5–10% kann der Tumorthrombus die Vena cava erreichen und sich in ihr sogar bis in den rechten Vorhof ausdehnen. Rückschlüsse auf den Malignitätsgrad des Tumors sind dabei nur bedingt möglich. Von Brantley (1985) wurde der Fall eines Cavazapfens bei gutartigem Angiomyolipom beschrieben [2]. Erstaunlich ist, daß trotz des frühzeitigen Einbruchs des Tumors in das venöse System eine Fern- oder Regionalmetastasierung nur in knapp 40% der Fälle zu registrieren ist. Die Prognose beim Nierenkarzinom mit Befall der Vena cava ist daher auch nicht so schlecht, wie zunächst angenommen werden müßte.
