Zusammenfassung
Gestufte Intensitäten von Lichtcoagulation wurden als ein Modell benutzt, die Netzhaut von Rhesusaffen (macaca mulatta) zu verletzen und den Prozeß der Netzhautheilung zu studieren. Die in dieser Arbeit untersuchten mäßig intensiven Läsionen zeigten ophthalmoskopisch ein Areal grauer Netzhautverfärbung, umgeben von einem graugelben Ring. Bei licht- und elektronenmikroskopischer Auswertung bestand ein Schadensgebiet in der äußeren Netzhauthälfte und in der Aderhautcapillaris. Die Bruchsche Membran blieb erhalten. Während der Heilung kam es in der Netzhaut zu einem differenzierten Reparationsmuster, getragen von Pigmentepithel, hypertrophierten Müllerzellen und einigen Photo-receptorzeilen. Eine neue Adhäsion zwischen Netzhaut und Aderhaut entwickelte sich nicht. In der Aderhaut kam es zum Verschluß von Capillaren.
Wenn diese Ergebnisse auf den Menschen übertragbar sind, sind einzeln gesetzte mäßig intensive Lichtcoagulationsherde ungeeignet für die Ablatioprophylaxe. Behandlungserfolge von Formen der Retinopathia centralis serosa durch Lichtcoagulationsherde dieser Größenordnung würden ohne Vorhandensein einer chorioretinalen Adhäsion Zustandekommen. Eine Erklärungsmöglichkeit für solche Behandlungserfolge wird angeboten.
Summary
Graded intensities of xenon arc photocoagulation were used as a model to damage the retina of rhesus monkey (macaca mulatta) and to study the process of retinal repair. Moderately intense burns evaluated in this paper ophthalmoscopically consisted of an area of gray retinal discoloration surrounded by one grayyellow ring. Light and electron microscopic examination showed damage involving the outer retinal half and the choriocapillaris. Bruch’s membrane remained continuous. During repair a differentiated pattern of retinal healing developed provided by the retinal pigment epithelium, hypertrophic Müller cells, and some photoreceptor cells. There was no formation of a new adhesion between retina and choroid. Within the choroid, there was obliteration of capillaries.
If these results are applicable to man, single-placed moderately intense photocoagulation burns are inadequate as a means to prevent retinal detachment. Successful treatment of forms of central serous retinopathy by photocoagulation lesions of this magnitude would occur without the presence of a chorioretinal adhesion. A possible explanation for such successful treatment is suggested.
