Chirurgische Möglichkeiten der Therapie tachykarder Arrhythmien (Operative Methoden, Risiken und Ergebnisse)

Zusammenfassung

Bei den Tachykardien, welche bei medikamen töser Therapieresistenz für eine chirurgische Behandlung in Frage kommen, handelt es sich vorrangig um Rhythmusstörungen, die durch einen sogenannten Reentry-Mechanismus ini tiiert und aufrechterhalten werden (3, 6, 9, 11, 12, 15, 32). Dabei haben sich im wesentlichen zwei Gruppen von Arrhythmien herauskristallisiert, nämlich Tachykardien, welche im Zusammenhang mit einem akzessorischen atrioventrikulären Überleitungsbündel der Wolff-Parkinson-White-Spezifität auftreten, zum anderem um ventrikuläre Tachykardien, die im Gefolge einer koronaren Herzerkrankung mit Myokardinfarkt und linksventrikulärem Aneurysma beobachtet werden. In diesem Zusammenhang befassen wir uns mit der Frage, wie durch morphologische Manipulation eine unter Umständen lebensbedrohliche Tachykardie therapeutisch beeinflußt oder sogar beseitigt werden kann.
