Zusammenfassung
Klinische Studien haben gezeigt, daß bei Patienten mit chronischem Vorhofflimmern ohne Herzklappenfehler in Abhängigkeit von weiteren prädisponierenden Faktoren das Risiko arterieller Embolien auf eine Rate von bis zu 18%/Jahr steigen kann. Wegweisend sind echokardiographische Kriterien. Als klinische Faktoren gelten die Herzinsuffizienz, arterielle Hypertonie und Thromboembolieanamnese als prädisponierend. Bei Patienten mit mäßigem Risiko für thromboembolische Komplikationen kann Aspirin in einer Dosierung von ca. 325 mg täglich eingesetzt werden. In geringerer Dosierung und bei Patienten mit einem Alter von über 75 Jahren ist Aspirin dagegen wirkungslos. Bei Patienten mit einem höheren Thromboembolierisiko sollte eine orale Antikoagulation erfolgen, sofern keine Kontraindikationen für eine solche Behandlung vorliegen. Die Low-dose-Antikoagulation ist nach ersten Untersuchungen der traditionellen Cumarin-Behandlung wahrscheinlich als gleichwertig zu bezeichnen. Bei Patienten mit hohem Thromboembolierisiko sollte aber in jedem Fall wie bisher im traditionellen Bereich antikoaguliert werden. Bei Patienten ohne Hinweise auf eine strukturelle Herzerkrankung (lown fibrillation) und normal großem linken Vorhof kann auf eine gerinnungshemmende Behandlung verzichtet werden.
