Zusammenfassung
Der Mitralklappenprolaps (MKP) ist mit einer Inzidenz von vier bis sechs Prozent in der Allgemeinbevölkerung eine der häufigsten Herzklappenanomalien des Erwachsenenalters (9). Eine besonders schwerwiegende Begleiterscheinung eines MKPs stellen kardial-embolisch bedingte zerebro-vaskuläre Ereignisse dar. So sollen bei Patienten unter 45 Lebensjahren etwa 30% der TIA und Hirninfarkte auf einen MKP zurückzuführen sein (4). MKP-Patienten leiden bemerkenswert häufig unter einer Migräne (1,12). Die fokal-neurologischen Ausfälle bei Patienten mit einer “hemiplegischen” Migräne (6) ähneln der TIA-Symptomatik, die bei einem MKP auftreten kann. Darüber hinaus soll das Risiko, einen Hirninfarkt zu erleiden, bei Migränikern, insbesondere bei solchen mit einer hemiplegischen Migräne, höher als in der Normalpopulation sein (13). Migräniker weisen nicht selten schon im kopf-schmerzfreien Intervall (10) eine Störung der Thrombozytenfunktion im Sinne einer gesteigerten Plättchenadhäsivität und -aggregation auf (11). Eine solche Thrombozytendysfunktion, die allgemein als Risikofaktor einer zerebrovaskulären Komplikation gilt, soll auch bei MKP-Patienten nachweisbar sein (7,14–16). Beide Erkrankungen zeichnen sich noch durch eine Reihe anderer Gemeinsamkeiten aus.
