Zusammenfassung
Präcancerosen und Malignome der Haut sind häufig lichtlokalisiert und licht ausgelöst. Auftreten und Häufigkeit hängen ab von Ausmaß und Frequenz starker Lichtexpositionen der Haut, aber auch vom Pigmentkleid und der Effektivität cellulärer Reparaturmechanismen. Zudem spielt die immunologische Selbstüberwachung eine Rolle. Modellkrankheiten, wie Albinismus und Xeroderma pigmentosum, haben wesentliche pathogenetische Einblicke ermöglicht. Die Erfassung des individuellen Risikos für lichtinduzierte Spätschäden zeichnet sich ab. Die Messung der cellulären Repairaktivität, der persistenten Dimere, der spontanen und der lichtinduzierten SCE und der Persistenz eines kräftigen Erythems führen zur Absteckung eines sog. „Risikospektrums“, welches die Basis darstellt zur individuellen Beratung und zur prophylaktischen Maßnahme.
Summary
Precanceroses and malignancies of the skin are UV-induced and therefore usually found in lightexposed areas. Occurence and frequency are as well dependent on the intensity of exposure to sun light as on pigmentation and the effectiveness of cellular repair mechanisms. In addition immunological self surveillance is playing an important role.
Deseases like albinism and xeroderma pigmentosum have given substantial aspects of the pathomechanism. The comprehension of individual risk of light induced damage is outlined. The measurement of cellular repair capacity, of persistent dimers, of spontaneous and of light induced SCE and the persistance of a marked erythema define a spectrum of risk factors, which gives a basis for individual risk evaluation and active prophylaxis.
