Zusammenfassung
Nach Ausschluß oder Beseitigung intrakranieller Raumforderungen und epileptischer cerebraler Reizzustände wird das schwere Schädel-Hirn-Trauma durch Überwachung der noch vorhandenen und Unterstützung oder Ersatz gestörter zentral-nervöser Funktionen behandelt. Wesentlicher Teil dieser Intensivtherapie ist die Kontrolle des Wasser- und Elektrolythaushaltes [21, 22]. Bei schwer Schädel-Hirn-Verletzten, d.h. bewußtseinsgetrübten [10, 29] Patienten fehlen nicht nur Durst und spontane Wasseraufnahme [5], vielmehr ist bei Schädel-Hirn-Verletzungen auch mit einer zentralen Störung der Reglerkreise des Wasser- und Elektrolythaushaltes zu rechnen, die in komplizierter Verbindung von zentral-nervöser, insbesondere diencephal-hypothalamisch-hypophysärer Funktion mit peripheren Rezeptor- und Effektor-(Niere-)Funktionen und endokrinen Organen [7, 9, 7] den osmotischen Druck in alien Körperflüssigkeiten mit Ausnahme der Exkrete konstant auf 289 ± 8 (Mittelwert ± 2 SD) mmol/kg hält [8].1
