Correlation of Intrapartum Changes in Fetal Heart Rate with Fetal Blood Oxygen and Acid-Base State

Abstract

Previous work has established a correlation between depression of the newborn (shown by an Apgar score of 6 or less; Apgar et al., 1958) and the presence of certain characteristic changes in fetal heart rate during labor (Caldeyro-Barcia et al., 1966, 1967; Hon, 1967). These changes are:

  1. 1.

    The mean FHR between contractions is higher than normal (Brady et al., 1963), average 165 beats/min against 143 beats/min in the normal condition (Caldeyro-Barcia et al., 1966, 1967).

  2. 2.

    Each uterine contraction causes a type II dip, i.e., a transient fall in FHR which reaches the maximum 30 to 60 seconds after the peak of the contraction (Fig. 1). The newborns are usually depressed when more than 20 such events occur prior to delivery (Caldeyro-Barcia et al., 1966).
