Die transkutane und laparoskopische Laseranwendung zur Behandlung ausgedehnter retroperitonealer Lymphangiome im Kindesalter

Summary

Retroperitoneal lymphangiomas are rare congenital vascular malformations. They cannot always be completely excised and are associated with high recurrence, complication and morbidity rates. We therefore utilize an alternative treatment concept in some cases. We excise the cystic types laparoscopically with a Nd: YAG laser (wavelength 1064 nm). Residual tissues are percutaneously managed by interstitial laser therapy under MRI monitoring. The high soft-tissue contrast of the MRI enables exact positioning of the laser fiber. The examination is thermosensitive and provides online and noninvasive demonstration of the interstitial tissue coagulation. We have treated four infants laparoscopically and three other children percutaneously.

Zusammenfassung

Retroperitoneale Lymphangiome sind seltene angeborene vaskuläre Fehlbildungen. Die vollständige Exzision ist nicht immer möglich, die Rezidiv-, Komplikations- und Morbiditätsraten sind beachtlich. Deshalb wenden wir in bestimmten Fällen ein alternatives Behandlungskonzept an. Die zystischen Formen exzidieren wir mit Hilfe des Nd: YAG Laser 1064 nm laparoskopisch. Residuen werden perkutan unter MRT-Kontrolle interstitiell gelasert. Der hohe Weichteilkontrast des MRTs ermöglicht eine exakte Positionierung der Laserfaser. Die Untersuchung ist thermosensibel und zeigt die interstitielle Gewebekoagulation online und nicht invasiv an. Wir haben vier Säuglinge laparoskopisch und drei weitere Kinder perkutan behandelt.
